DPA Dissertation Forms and Process

 

Please ensure all forms are fully completed (excluding signatures) and submitted via email to pa@valdosta.edu. Include email addresses of all committee members in email.
 
Upon receipt, the form(s) will be routed through DocuSign for the necessary signatures by the PA Office.
 
All parties will receive a copy of the form(s) directly from DocuSign once completed.

 

1. Forming committee:

  • Dissertation Committee Form

    *All committee members must hold approved graduate faculty status. This status can be verified at the following link: Graduate Faculty Status Verification

    *If any committee member does not currently hold Temporary Graduate Faculty Status, please have them send their updated CV to pa@valdosta.edu for TGFS approval. The committee member will receive a notification from the Graduate School once their Temporary Graduate Faculty Status has been approved.

     *If the research member line is blank, then the chair of the committee will serve as the chair and research member.

    *Individuals serving as the research (Methodologist) committee member are required to submit an updated CITI certificate showing completion of the IRB Basic course. The following link provides access to the CITI training necessary for this role: CITI Training
